SAM’S PLACE is a Texas Health and Human Services (HHS) community provider of Texas Home Living (TxHmL) and Home and Community-based Services (HCS) program services located in Wharton Texas, servicing Wharton, Austin, Matagorda, Colorado, Fort Bend, and Waller Counties. We provide individualized supports and services to people with Intellectual and Developmental Disabilities (IDD). Our services include residential assistance, day habilitation, specialized therapy, respite care, and nursing.
